Letter format Bank account Transfer from one Branch to another

I would be obliged if you can please transfer my account maintained with your branch to your bank's .......... branch. This would ease up the maintenance of the account at my end. Appropriate charges may kindly be debited from my account in this regard for transfer of the account. 

I am enclosing herewith a copy of the Leave and License Agreement of my current residence for your records and verification purposes.
