I, (Name of the Father), aged ... years, son of (Name), having Indian Passport Number .................., Date of Issue: ................, Place of Issue: ................, presently residing at (Address), do hereby solemnly affirm and declare that:

1. Ms. (Name of the Daughter) is my daughter, whose date of birth is ............ and place of birth is ..........;

2. She is the holder of Indian Passport Number .................., Date of Issue: ................, Place of Issue: ................ and is presently residing at (Address);

3. I do not have any objection towards employment of my daughter in the company i.e. ................. Limited, CIN: ............................, having its registered office at ..............................;

4. I do not have any objection in issue of a separate employment visa to my daughter;

5. All the contents mentioned hereinabove is true and no part of it is false. Nothing has been concealed therefrom;

6. I understand that I will be liable to be prosecuted, if any part of this document is anyway found to be incorrect or false;

7. I also affirm that in the case of court case(s) arising due to the issue of a visa to my daughter, I would be solely responsible for defending the case(s) and not the Embassy of India, .............;

8. I hold myself fully responsible for the authenticity of this document and the information given therein and would be solely responsible for the consequences should this document and the information given therein found to be untrue;

9. I shall indemnify the Embassy of India, ............. towards any claim(s), expense(s), loss(es) or damage(s) in case the information given hereinabove in this document is not found to be genuine at any stage.
