Affidavit for Change of Appearance in Indian Passport (Format)

You should have your passport updated in every few years to change the photograph in it, especially if your appearance has changed in any major way. It will help you avoid trouble at the airport while going abroad or travelling by air otherwise during the passport verification stage.

While applying for the reissue of the passport for change of appearance, one of the documents that may be asked from you is to produce an affidavit, wherein you declare that your appearance has changed and you are the same person as mentioned in the passport.

Here is a template of an affidavit for change of appearance in the Indian passport.

3. That my appearance meanwhile has changed since the issue of the above mentioned passport of mine;

4. That my appearance has changed due to (mention here the reason viz. Moustache, Beard, Long Hairs on head, Turban, Hijab, Sideburns, Scarf, etc).

5. That my photo to match my current appearance is affixed herein below;

6. That I am the same person as shown in my previous passport and the photo below.
