Affidavit - Address Proof for Vehicle Registration Driving License

The applicant for the vehicle registration or driving licence is required to submit a self attested copy of address proof as per the categories of documents specified in the Motor Vehicles Act viz. Electoral Roll, Life Insurance Policy, Passport, Employer Certificate, etc.

If the applicant does not have the specified category of address proof document as mentioned above, then he will have to provide the copies of any other categories of document other than the ones specified in the Motor Vehicles Act viz. Voter Id Card, Aadhar Card, Rent Agreement, Latest Telephone Bill, etc. But he will also have to provide an affidavit to this effect.

I/We, (Name of the Applicant), aged .... years, son/daughter of (Name of the Father), presently residing at ....................................... do hereby solemnly affirm and declare as under:-

1. That I/we have enclosed (Name of the Document) as the address proof;

2. That I/we do not have any other address proofs i.e. Electoral Roll, LIC, Passport, etc. except the above-mentioned document;

3. That the enclosed address proof is true and genuine and I/we shall be fully responsible for submitting any fake document.

I/We, (Name of the Applicant), age ... years, son/daughter of (Name of the Father) presently residing at (full address) solemnly declare and affirm that the statements made in the above affidavit and information provided in the application are true and correct to the best of my/our knowledge and belief and that nothing material has been concealed therefrom.

That in case of any concealment or misrepresentation, legal action may be taken against me/us. Such action can be taken under Sections 182 and 415 of the Indian Penal Code (IPC) read with Sections 417 and 420 of the Indian Penal Code (IPC) as the case may be.

    An affidavit is a type of proved statement or showing, or in other words, it contains a verification, which means that it is made under pledge on disadvantage of lie, and this serves as evidence for its truth and is required in court proceedings.
